POWER TYPE	WEBSITE FORM
Type 1: Spherical (SPH) Only Power 60% of people have only spherical power (SPH) with no cylinder (CYL) or axis (AXIS)
Type 2: Spherical with Cylinder (CYL) Power 30% of people also have CYL power. The cylinder is an angular error and is defined by two variables: CYL and AXIS. If you have CYL you have AXIS, otherwise none.
Type 3: Bifocal / Progressive Power This is for people who have trouble seeing far and near. 10% of people do it.And only if you want both powers in one spec should you go with this option. This power has a new variable "ADD" (Additional Power), which indicates a near-spherical power. Additional power means power required over distance power. If you just want to be close to power, you can type ADD in Type 1 above. If you only want ADD Power in the specification, you can choose Type 1 or 2 above again without ADD Power
